Medical Records
Dear Patient,
The Ohio Ear Institute closed permanently on October 2025. All patient records have been archived and are available till October 2031 via our custodian of records.
There will be a charge for providing the complete medical records requested for yourself or to be given to another entity of your choosing. The amount is set and allowable by the state of Ohio, and only available after you provide us with a written signed request (see form below).
HIPAA Release Policy
To obtain your medical records from the Ohio Ear Institute, and protect your health privacy information, we require you to provide us with our signed HIPAA release form.
Please fill out the form, which must include your current mailing address to which we will mail a digital copy of your records.
We require an alternate way to contact you (cell phone or email) to provide you the password to your encrypted records copy.
There is a flat $35 processing fee to provide complete medical records (please see Ohio Department of Health Records fees ).
We will maintain your records as required by law, however as the practice is now closed, be advised it may take some time to retrieve and send your records. We appreciate your understanding.
To expedite getting your records, please download the HIPAA form on this page below, fill it out as instructed. To expedite the process email it to us at records@ohioear.com. You can scan it into a PDF or take a picture of both sides and email it to us.
After June 1, 2026 you can call Beth at (614) 759-8811 ext. 301, to ask any question about your records, or provide her the signed HIPAA form directly.
Complete Records Payment
We will mail out your complete medical records to the address you specified, after receiving your payment which we will obtain by contacting you at the number you listed on your HIPAA release form.
