Medical Records
Dear Patient,
There will be a charge for physical copying medical records requested for yourself or to be given to another provider. The amount is set and allowable by the state of Ohio. If you provide us with a written signed request from you, will can fax them directly without charge.
HIPAA Release Policy
To obtain your medical records from the Ohio Ear Institute, and protect your health privacy information, we require you to provide us with our signed HIPAA release form.
Please fill out the form, which must include your current mailing address, the contact information for your new health provider to whom you would like your records faxed (you must provide the fax number to us)
Please specify if you require your most recent clinically relevant records (faxed to your provider for free till Feb 1, 2026), or a complete copy which includes your billing records. The later option can be several hundred pages and as such can only be provided on an encrypted flash drive. There is a flat $35 processing fee to provide complete medical records (please see Ohio Department of Health Records fees ).
We will maintain your records as required by law, however as the practice is now closed, be advised it may take some time to retrieve and send your records. We appreciate your understanding.
To expedite getting your records, please download the HIPAA form on this page below, fill it out as instructed, and email it to us at records@ohioear.com. You can scan it into a PDF or take a picture of both sides and email it to us.
After February 1, 2026 your records can be obtained by calling Beth at (614) 759-8811 ext. 301, to make that request and provide her directly with the signed HIPAA form.
Complete Records Payment
We will begin mailing out complete medical records starting December 1, 2025 to the address you specified, after receiving your credit card payment which we will obtain by contacting you at the number you listed on your HIPAA release form.
