Dear Patient,

Per your registration preferences when you established with our medical practice, you “Opted-In” to receive communications from us. SMS consent is not shared with third parties for marketing or ANY other purposes. From time to time we will contact you via text / SMS to notify you of your appointment time, location, pre-procedure instruction, and respond to billing questions. You may Opt-Out at anytime by responding “STOP”, and we will remove you from further communications.

Should you decide to re-enroll with SMS communications, you will need to call the office at 614-891-9190 to resume texting communications to change your preference.

Who personal information is shared with

Per HIPAA guidelines, your personal information is not shared with any third-party or anyone other than the individuals you designated on your patient enrollment form. And never through SMS.

What personal information is collected

From SMS communications with you, no personal information is collected or retained, other than your contact information to provide for your healthcare needs.

How personal information is used

Your personal communication information (name, cell phone) will only be used, per your written registration request, to receive notifications from us. We will not respond to any medical questions in this format, or collect any other information (date of birth, social security, insurance plan etc) via SMS, so please do not share it, instead call us at 614-891-9190 to update any personal information.
