Surgery Policy

Dear Patient,

For the procedure you discussed with your Surgeon, we will contact your insurance company and obtain the necessary approvals, initially at no additional charge, however this is very time consuming, and repeated rescheduling may incur a cost. To that end, we require a $250 surgical deposit, to hold your surgery date, and perform all the surgery scheduling tasks. This deposit will either be refunded fully after surgery if the patient has already met their deductible, or applied to their remaining deductible balance.

Prior to your surgery we will verify with your insurance company what your remaining deductible for the year is, and require no later than the week prior to surgery either $500 or 50% of the amount the patient will be responsible for to your surgeon, as determined by your insurance company (whichever is greater).  Your remaining portion assigned to you by your insurance company if any, will be due on your next office visit once that amount is finalized. Any over payments, will be promptly refunded back to you.

Missed Appointments

We understand the unforeseen events can occur, requiring you to reschedule or cancel your appointment. However as a courtesy to our other patients, we require that the patient/guarantor notify us of this one (1) full business day prior to the appointment, or we will charge a fee of $50.00 for each provider appointment you fail to keep. If you are running late, please show up for your appointment regardless and we will try to work you in. Medical or family emergencies will be given consideration.

We attempt to remind patients of their appointments by text , or the patient portal, but it is the patient’s responsibility to remember appointment dates and times.  The fee will be applied for both missed medical appointments and/or audiologic testing.

Self Pay

If you do not have health insurance or you have a rider that excludes ear problems, The Ohio Ear Institute will discount your bill 30%.  The full amount will be due at the time of service.

Collections

Past due amounts payment for prior services if any, are to be made at the time of service.  Accounts that are 90 days past due will be forwarded to a collection agency unless payment arrangements have been made with our business office.  If your account is forwarded to a collection agency we will ask that you seek medical care from another physician until payment in full has been made.  We will notify you via certified mail that after 30 days, you will need to seek a new provider.

Medical Records

There will be a charge for physical copying medical records requested for yourself or to be given to another provider.  The amount is set and allowable by the state of Ohio. If your provider submits a written signed request from you, will can fax them directly without charge. You can obtain your records for free by requesting them via your patient portal.

Form Completion

There may be a $30.00 charge for form completion.
