Ohio Ear Institute Patient Records Policy
Dear Patient,
To obtain your medical records from Ohio Ear Institute, and to protect your protected health information, we require that you provide us with a signed HIPAA release form.
Please specify if you require your most recent clinically relevant records, or a complete medical record, which includes all of your billing records. The latter option can be several hundred pages and can only be provided on an encrypted flash drive. There is a flat $35 processing fee to provide complete medical records. This amount is set and allowable by the State of Ohio. Ohio Ear Institute only accepts a credit card payment.
We will begin mailing out complete medical records starting Nov 30, 2025 to the address you specified, after receiving your credit card payment. We will contact you at the number you listed on your HIPAA release form to obtain payment.
If you do not need a complete medical record, upon receipt of a written signed request from you, we can fax records directly without charge.
We will maintain your records as required by law; however, as the practice is now closed, be advised it may take some time to retrieve and send your records. We appreciate your understanding.
To expedite getting your records, download the HIPAA form on this page, fill it out, and email it to us at records@ohioear.com, or fax the form (both sides) to 614-839-9174.
After November 30, 2025 your records can be obtained by calling Beth at (614) 759-8811 ext. 301, to make that request, and provide the signed HIPAA form.
